F14 Waiver for Release of Student File Information
In accordance with the Family Education Rights and Privacy Act of 1974 (FERPA), Thiel College may not release information concerning your attendance, academic performance, disciplinary matter, or other private information normally kept in your student file to third parties, including your parent(s) or legal guardians(s). To access the above noted information, third parties must either prove your financial dependency upon them, or you must grant a waiver of the rights given you.
Because this act gives you, the student, control over the release of information, information concerning your institutional record and status will not be sent to anyone without your consent. This release has some exceptions and those include suspension, dismissal, expulsion, incidents involving alcohol or drugs, and general institutional discretion.
You are therefore requested to indicate below whether you waive or do not waive the provisions of the Family Educational Rights and Privacy Act of 1974 pertaining to the release of your student file information to third parties.
